It can reduce your risk of chronic health conditions; such as heart disease, type 2 diabetes and cancer, as well as help. Allied health and therapy services funded through Australian Government Department of Health Commonwealth Home Support Program (CHSP). Community Health has been shown to have an independent effect on improving health status, reducing health inequalities, achieving better health outcomes. Central Highlands Rural Health provides a broad range of health promotion activities and initiatives to the communities in the Hepburn Shire. Provides assessment and therapy services for children and adults with communication and/or swallowing difficulties. Social Work is available at all sites and provides a range of support and information services across the lifespan.  This includes working with school-aged children, families and through to end of life. Smiles 4 Miles is an initiative of Dental Health Services Victoria (DHSV)  which aims to improve the oral health of children and their families across Victoria. Central Highlands Rural Health is pleased to offer local primary schools a comprehensive Puberty Education program for grade 5 & 6 students. Podiatry deals with the prevention, diagnosis, treatment and rehabilitation of medical and surgical conditions of the feet and lower limbs. Health Promotion is defined as a 'process of enabling people to increase control over, and to improve their health'. Adult dental services are available to holders of a current Health Care or Pensioner Concession card.  Psychology services for individuals at Clunes, Creswick, Daylesford and Trentham. Treehouse is a supportive and safe social place for people with a palliative condition, dementia or any chronic illness. Physiotherapists assess, diagnose, plan and manage the care of patients across a broad range of areas with musculoskeletal, cardiothoracic and neurological injuries and conditions. Our Oral Health services include:Early Childhood Oral Health Program. Our Alcohol and Other Drugs Services include brief interventions, community withdrawal and medium to longer-term counselling.
